New Patients

Thank you for becoming a new patient of Rocky Vista Health Center. If you have schedule an appointment with us, please check your email to complete the new patient paperwork packet. The following steps will ensure you have all the information needed prior to your first appointment with us.

If possible, complete your new patient paperwork before your appointment. Our staff will email you new client paperwork. Below, please fill out the authorization for our staff to receive your medical records.


It is helpful for you to have the time to complete the forms. Try your best to complete your medical records including hospitalizations, family history of illness, etc.


Please gather all your medical records. Request your medical records from anyone who has been recently involved in your care. Make a copy of these records to keep at home and fax a copy to us at 720-875-2877 before your appointment if possible. Or bring all records and paperwork to your appointment. Tests may include mammograms, bone density scans, colonoscopies, and blood work for cholesterol and diabetes.


What’s in your medicine chest — and a kitchen cabinet? When gathering your medical records, be sure to include a complete list of medications. This includes more than your prescription drugs. You also want to include over the counter medications that you take on a regular basis, such as acetaminophen, ibuprofen, aspirin, and cold medications. Also, remember to list any vitamins or dietary supplements. It is ideal to bring your medications in their prescription bottles to an appointment at least once a year, if not every time for a review.

Is history not complete? All medical history does not have to be completed by the time of your appointment. However, a complete medical history enables us to serve you better.

 

Please bring your insurance card and driver's license to your appointment. Please use the below form to send your medical records to our staff. New patient paperwork will be emailed to you. We look forward to working with you.
